1. When the blades came off you lost at least one of the spacers/washers that go between the blade grips and the head dampeners.

2. Look for "oil tolerant" or "surface insensitive" thread lock. I have had this happen to me a few times. No amount of cleaning with alcohol seemed to work. I haven't had an incident since I've been using the oil tolerant thread lock.

1.) Always whether heli is new or used ensure to take time to apply loctite and glue where the manual states it is needed.

2.) Never trust someone elese build not matter what, inspect and re apply loctite where it is needed.

3.) Always do pre flight checks prior to your day of flying.

4.) Know your heli intimetently, read manual front to back and ensure to do adequate maintenence.

5.) Buy good stuff once save thousands over years, this is not a cheap hobby and dont think it can be. Your safety and the safety of others can and will be at risk without your knowledge and smart choices.

Check the feathering shaft for being true. Take the blade off the other side and turn the screw at that end of the shaft.As you turn the screw if you see any kind of wobble or deflection in the shaft at either end change the shaft otherwise blade tracking will be near impossible.

Since the blade is still attached to the blade holder, I think the screw and the bearings are still there. Carefully remove the blade, and see if it still there. There are washers and bearings there also. Remember the order and orientation of each. Very important. Also, note that you might to add a little grease to the bearings while you are at it, and when you apply Pickford, you only need a little dab, not a drop. Remove excess with a tissue before screwing in. Push the screw in first and then apply locktite to avoid putting any on the bearings. Get blue thread locker. Color indicates strength. The strength code is 262 as was mentioned earlier. My little red bottle says Z-32 I believe (rubbed off).

This is the exact same thing that happened to my heli except that in the video the heli is airborne and mine was still on the ground at about 40% throttle!

Like Cassat said, I think theres a screw missing ...

Let me try to take out the blades from the holder and see what's going on ...

Based on your picture the feathering shaft looks like it's still there. Either way, you need to remove the blades from the blade holders to see what's going on. You're also going to need 2 hex drivers (allen keys will work in a pinch). You'll need 1.5mm.

Basically I am going to have put in a new feathering shaft (align part # h45021a).
