Happened to me with a battery which was a bit puffed up to start. Was charging at 1C only with balance lead plug-in. Note that I was downstairs, and the smoke detector didn't start because the particles in the air are very small (The smoke detector will go off with something on the stove, BBQ outside with a window opened...). There was fine black dust everywhere... And yes, I have learned my lesson, and use charge bags now. Good thing I had a granite piece under the battery.
