i know this is a bit vague and im not pro but what about sticking that same pen into the hole where the manifold goes and trying to pry out the sleeve?..or is that what you did? any chance of heating it up?

heating the case causes it to expand which usually does it.

sleeve removal.

turn the crank over and see if the piston pushes up the sleeve.i usually grab the sleeve with needle nose pliers wrapped in rubber.is there a lip at the top?if so grab it on the oter ridge.pull lightly, but make sure you have a firm grip.

Sleeve

Tie straps through the exhaust port. One to start, if it cuts the strap add one more at a time.

Heat gun to the block can help and sometime heat "cycling" can break it free. (heat block/ drop in freezer for 10 minutes and repeat with tie straps)

Tie straps

Put the tie strap (good ones, fairly thick, no dollar store junk) in the exhaust port with the piston at the bottom of its travel. then just turn the crankshaft so the strap gets caught between the piston and the sleeve, then just continue to turn, the sleeve should start to rise to the point that it can be gripped safely with soft covered pliers.

DO NOT USE THIS METHOD WITH ANYTHING OTHER THAN TIE STRAPS. Anything bigger and tougher than a tie strap or two will damage the piston.

Got this tip from an old RC Car Action in an engine rebuild article.

If you are still having trouble understanding I can post a couple pics.
